Synopsis

The Phoenix Area Indian Health Service (IHS) is soliciting proposals from interested businesses to provide dietary consultant services under a non-personal services contract for the clients of the Desert Visions Youth Wellness Treatment Center in Sacaton, AZ, located on the Gila River Indian Community Reservation; virtual services shall also be conducted via satellite to the Nevada Skies Treatment Center located in Wadsworth, NV. All potential offerors shall be classified as a business entity. In addition, all potential offerors shall be subject to pre-security clearances prior to commencing services at the wellness center; the security clearance encompasses fingerprinting and a screen check through the Office of Inspector General (OIG) website. The anticipated period of performance is for a base year of twelve (12) months, beginning September 1, 2012 through August 31, 2013, with option to extend services for four (4) additional one-year option periods, or a total of five years. The Contractor shall meet all criteria contained in the Request for Proposal (RFP). The unit price shall be an all inclusive rate to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, including all other costs pertinent to the performance of the contract.
